I didn't forget, there is a reason. Nobody knows her last name. When she was one her parents got killed the same way Harry Potter's parent did. In fact Voldemort visit her house first. The only problem was Voldemort couldn't find Jazmin. She was under her own invisibility cloak. That's all you need to know for now.
