Sans glanced over at Alphys, and then Frisk, the two nodding back at him before he looked back at his brother.

All of Sans's life, he's had some difficulties with voices in his head. Or, just one simple female voice, always whispering things to him that he didn't want to hear. At first, he thought it was Chara, as Frisk had explained to him later in life.
