It's my first story...meh be as harsh as you feel is require The "appendages" are like that of the photo up there but are organic and come from our back...it's like walking but without legs. Now that I read over the first part I feel like I should explain...the creature is suffering from ptsd, berserker-type moods?, schizophrenia, and mpd/DID The mouth is a mix of goblin shark and a crocodile...think of that monster from pacific rim you know the knife nose one? But with conical teeth and a obscene amount of bite force. The arms and legs are roughly like that of a humanoid in shape but that's where the similarities end as it has the tail of a utahraptor and claws like that of a therizinosaurus but the powerful legs of a Tarabosaurus.
