Keith struggles with depression and anxiety while having to defeat Zarkon. Pressure is building on his shoulders while more people are depending on him to save Earth. To relieve stress, he self harms by cutting himself. Though it makes him weaker, it helps him. Plus, he has a huge crush on Lance while on the other hand, Lance seems oblivious.
