It just means that when someone is faced with torture or something similar to it you will gain a quirk. It's pretty rare so he doesn't use it much. This quirk allows the owner to have a certain thing happen to him based on his emotions. Deku had gone through a Femboy phase so he just goes with it now, no matter what people say or think and somehow got a harem along the way. He has grown mentally and physically tough so it's hard to get to his heart. Will his harem the consist of his classmates be able to break down the layers to this badass femboy's heart or not. Izuku lives with his father as his mother was killed the hero killer, stain, when he was 10. But his father abuses him whenever he feels like it. He didn't care where or how bad it hurt.
