It's been almost six months since Hazel lost the love of her life, Augustus. Hazel has become extremely weak and the cancer is slowly taking her. Just as she thinks everything is hopeless, and her time is quickly approaching, she finds out there is one last hope. Surgery. It would be the biggest one she'll have, hopefully getting rid of the cancer once and for all. A lung and thyroid gland transplant. A very risky option. 
Hazel mourns over Gus's death and soon finds out he left her one last gift that could change her life forever.
