If you refuse because you would never want to inflict pain on your special person then you feel pain. You are given a shock for every 1 minute that passes without the person inflicting pain. The more you refuse the more you get shocked and the voltage increases. You have a 30 minute time window, which means at a minimum the person is inflicting 30 wounds.
