Tsunade thought she died, she did have a heart attack.
The Godaime hokage died at 1:30 pm due to a heart attack caused by stress that was not supossed to be happening at the time.

But as fate would have it, she became pregnant and realizing that those elders would take her child whom would most likely gain the Sharingan, she fled from Konoha.
