Well the series of heart attacks of Mr Aizawa all start when the standardized medical testing of all UA students is pushed early after Midoriya's dealing of Gentle Criminal.

It was then, a fact so shocking was discovered. All of Izuku Midoriya's medical history only came from UA meaning that he never came to a clinic or hospital until UA. Meaning that the quirk before All For One are completely unknown.
