2- They weren't expecting an attack at the citadel but this was the aftermath
3- Nothing was different for the Dead, well other than the fact they had no clue where some of the swords had disappeared too. Except they weren't missing, they were alive while they themselves were dead.
4- What if Ochaco was too late, what if Deku made a wrong move and Shigaraki killed him
