A virus called The Pathogen has taken over the world. And Hazel Larussa and her best friend Jamie Diakos are in the midst of it. When Hazel's mother comes down with the virus and turns into what the infected are called - The Phantoms - Jamie and Hazel run away from their Phantom-infested city. 

They come across The Asylum, a safe haven for people who have fled their infected homelands. They meet the co-leader, Moon Lee. The three of them decide to scout ahead of the group and go to The Last City - where their government, The Rule, has inhabited itself. 

Along the way, they rescue Miguel Martinez and fight off Phantoms. Hazel has a huge secret she's kept from everyone, and it may cost her her life. But The Rule has a bigger secret - one they've kept from the world. When they reach The Last City, the secret is revealed and there is an uprising. 

But the friends have no time to join the uprising - they have to save Hazel from herself first.
