When the daughter of England (Arthur) and France (Francis) is protected time well, and hidden away she knows nothing about what she is. She is the nation of Nandia. And has quite a vibrant and thriving culture. At her first world meeting she falls for Germany (ludwig) but thinking it is unrequited, gets a disease known as hanahaki disease.

There are only two ways to cure the disease; it is if the person they love returns the feelings or it can be removed through surgery, though the feelings disappear along with the flowers. A new, deadly disease is spreading, and although necessary precautions are put in place, the nations don't show much concern. That is, until it's revealed they aren't immune to it either.
