She's a psychiatric doctor and a famous one at that, she helps her patients in her own way, and is good at taking care of them. 
She may put on a smile to brighten up your day, but it deceives all, it is a mask that she puts on to hide her loneliness and pain. 

Marcellus Zachary Knight, 26 years of age, as fierce as his name. He has had a lonely childhood, with nobody to look after him but himself. 
He has seen the dirty and dark parts of the world, he has endured it all, making his heart of stone, he kept going on. 
He thought of giving up on life from time to time. But add an autistic half-sister to his life, whom he adores more than his life, he calls her Pearl, because he thinks of her as a beautiful little pearl, which he has to protect by being the shell for his pearl. His sister became his reason to live, and he would go to the ends of the earth and beyond for her.

Just one event caused his joyful personality rolling off the cliff and replaced by a more serious, dominating and perhaps, the one which could make your knees go weak. In the journey of Love and Hate, they never quite realised how and when something clicked and when they had started to shield the unwanted feeling that suddenly started to blossom deep in their hearts.
