Forget me not disease; cause by unrequited love.
A disease where the victim watches the person they are in love with, fall for another. They slowly begin losing their memories about said person. It begins with small things such as the persons favorite color or birthday. As the victim's memories disappear, their health deteriorates. Then, at the final stage before death, they lose total memory of the one they love for good.
