Izuku Yagi was always in pain, simply going to school would see him getting punched, stabbed, cut, and getting lauched into walls by explosives. It kept on that path for 8 years until at age 14 he was kidnapped and then was tortured with several others until he awaked a very dark and destructive Quirk.

By the end off their third year of middle school, Izuku official become a machine, but one incident causes him to loss any shred of sanity he had left.
