What if Elena and Katherine were actually twins?
The inseparable twins did everything together ever since they were born but than Katherine gets pregnant out of wedlock and she is now disowned by her family but Elena won't let her go out alone so she goes with Katherine to England and they end up meeting the two original brothers Klaus and Elijah.
Klaus is still hellbent on sacrificing one of the Petrova doppelgängers, but who? He only needs one.
