When fighting, or should I say dying repeatedly, in that weird time loop, that scary magic guy told Strange that he gave up. On Strange's way out of the (rip in time?) he was suddenly, literally, blown into another reality. He wakes in his fifteen year old body yet his hands still shake and he remembers everything. This is where the story starts. But what reality did he travel to? And what will he do now that he's there?
