When u have turned 18 you will see a tether that leads you to your soulmate but the person has to be 18 as well for you to see it. If that's not the case you will see the tether when your soulmate turns 18. That tether won't disappear till you found each other. When you have kissed your soulmate for the first time after you see the tether their initials will show on the side of your neck.

It's not very painful at first, but the flowers get bigger and more frequently coughed up over a span of 2 years. If you have more than one soulmate (the max is 3), the flowers will be oleanders and will have two different colors the flowers can be. //Yes, I know that's not what Hanahaki disease is.
