From the age of 8 when you get a soulmate you also feel what they are feeling physically and mentally. But when they get hurt you get a terrible pain in your chest. And the worst pain the soulmate is having the worse the pain in your chest gets. Izuku Midoriya was born with an amazing and unique quirk that you only see every 2000 years as well as 2 other quirks. The design of the mark depends on both the soulmates eyes and quirk. The color is a exact replica of their soulmates eye color. And the shape and idea of the mark is dependent on their quirk. The amount of detail the mark has shows how good their mental health is. Say someone has a extremely breathtaking mark, absolutely stunning. Their soulmate is in mental agony. The more beautiful the mark, the more hurt their soulmate is. Not only will they have a mark connecting them to their soulmate, they can also feel each other's pain. Once both people turn 17, which can take years saying their could be a large age gap, they both can feel the physical pain something could inflict on them. Say one of them breaks their arm, the other wouldn't have a broken arm as well, they would simply be feeling the exact same amount of pain the other is feeling. Everyone gets marks at least once in their lifetime. It's just a matter of when. Marks can appear red, or black, the marks aren't set in stone. They're subject to change, they can disappear, change color, or even scar. A black mark means feelings on all sides are mutual, red means the love is unrequited, and well, a scar comes from a mark dying. Law is not one easily persued, he's never gotten a mark, and finds them to be, putting it frankly, a load of crap. How could some inexplainable mark on the skin determine love anyway?
