After Lucifer was defeated, Sam's demon-blood frenzy, powers and visions had supposedly come to an end. Only a few months later, Sam finds himself waking up from a terrifyingly vivid dream of a man plummeting to his death. The hunter brushes it off as a nightmare, until he starts having reoccurring forms of the dream every night.
When the Winchesters start to piece together an understanding of Sam's premonition, they come to the chilling revelation that Lucifer had escaped the Cage and is roaming among the living once again.
