As a child, Anna Grace Shramere immersed herself in an imaginary world. Guided by the nine "Rules of Imagination," Anna created hundreds of stories about a fantastic realm, filled with imaginary friends, secret superheroes, talking stuffed animals, flying cars, and parallel universes. But after being diagnosed with a life-threatening kidney disease, she finds herself thrust into the frightening world of hospitals, infusions, and tests. The now-teenage Anna Grace finds her refuge by immersing herself in the fictional stories of her childhood.

As her health deteriorates, her new stories grow darker. Anna Grace gives her disease to one of her youngest imaginary characters, and villains threaten their once-safe world. As Anna Grace fights for her life, the fate of her characters and that of the Underground City hang in the balance.
