In a world where the darkness within oneself is a sickness, and becomes a concrete thing, destroying the organs, causing tissues to go gangrene , poisoning and mutating the body. A particular set of surgeon, if payed the right amount, are willing to search the darkness within the body, remove it completely, promising to then burn it in front of your very eyes. 
Since the most evil and despicable acts are done consciously, there's a catch: the darkness within takes form only when the person is conscious, therefore, the patient must be fully aware and awake during the surgery. 
Not the pubblic nor the the Print or even the government knows what happens after this darkness is removed. It does not help the fact that the surgeons categorically refuse to explain what happens before, during and after the operation, not even saying what or how a person looks like once everything is done. Some of them even denying having performed said surgery.

But what Meredith doesn't yet understand, is that the Second Stage has no windows, no entrances and no exits, for it accepts you when it wishes and releases you when it's finished.
