Anthony was born into the Italian Mob. His father used him as a "play toy" to get some buisness buddies to do him 'favors'. Anthony decided he had had enough, he was done being a "play thing" to his fathers buddies. So he ODed on Angle Dust, his drug of choice so he wouldn't feel anything during his 'sessions'.
