It happened right after the Daleks destroyed New York. The Doctor never saw the beam coming. It hit her straight in the chest. Her companion had just enough strength to pull her into her TARDIS, before he collapsed, with the Doctor in his arms. From there, well, you'll have to see.
And yes, I said "her" and "she". This is an AU, where the Doctor is a female, and her companion is male.

"The girl who waited and the last centurion," I tested their nicknames on my lips, hoping to get a small flicker of happiness, but I was only met by the weight on my heart doubling. His heart and mind opens to the screams of humans on Earth. One stands out, a little girl screaming, "PAPA". Then I heard it. A young girl screaming "PAPA!," a piercing scream, "PAPA AHH PAPA", louder than all the other cries of children or anyone in the world echoing in my mind. Heartbreaking. Even more than I could feel with two hearts. After walking for five minutes in the uniform tree line, I saw a tall fence surrounding an ominous brown building which somewhat resembled a prison. There were no windows, only wire mesh lining the entire building and hard concrete pillars in between. Surely the girl I'm looking for couldn't be in here? The Doctor uncovers a girl with a dark secret, will the Doctor save her or will she drag him down with her so they both burn. First they had to find Eleven. Now they have to find Twenty Two. A dilemma or difficult circumstance from which there is no escape because of mutually conflicting or dependent conditions. "a catch-22 situation".
