Naruto Uzumaki comes across a sacred blade dating back to the era of the Sage of Six Paths and encounters the legend himself, who sealed a fragment of his soul in the artifact millenia ago.
Changes to the canon story include (but are not limited to): Sasuke gaining a twin, the sword mentioned in the above paragraph, new arcs, reworked jutsu/ chakra mechanics, Naruto has a lot more common sense.
