The title of Mad Hatter has been passed down since the start of the Black Order. Only one is chosen upon the death of the previous. The Mad Hatter is a rare and powerful Exorcist and only the most power of Exorcist can take on the title. But with great power there comes a price. When using the Innocence of the Mad Hatter you surrender your body and your soul to it. You take on a different personality thanks to the Innocence. If your soul is too weak to handle the Innocence will take over and make you do things you wouldn't do on your own. Because when you carry the title of the "Mad Hatter" there's a chance you'll go mad.
  
Madeline Ashford took on the title has the Mad Hatter after the last Mad Hatter and her teacher was killed in battle. With his dying breath he made her his predecessor. Saying she would be the best Mad Hatter the Black Order has ever had. And since that day she's been doing all she can to keep her teacher's last moments of life a reality and living up to the name of Mad Hatter.
