After the hospital she worked at mysteriously goes bankrupt, Kurosawa Nanami is recruited to work at a rundown Tokyo hospital. When she arrives, she finds the hospital's atmosphere dark, cold, and grimy; as if someone-or something- has died.

Nanami is advised by her colleagues not to go on the forth floor of the hospital and enter room forty-nine. When she breaks her promise, she soon finds herself fighting for her life.
