Izuku Midoriya has a powerful quirk, but has a hard time controlling it. His quirk generates massive amounts of radioactive energy, and he has to where a special gas mask that dampens his quirk and filters the air he breaths out, as his quirk changed his biology so much that even without his quirk he still generates toxic amounts of radiation in his internal organs, and therefore the air he breathes out. Fortunately, Izuku is highly resistant to radiation even without his quirk, so he can handle it. But Izuku needs change, for his own sanity.
