Quite simple, honestly, Izuku has a quirk before One for All. One that not even he knows about. When he dies/is about to die, he is brought into a nightmare-ish dream of sorts. In this dream, he has to confront people from his past, and they try to convince him to give up. If they manage to persuade him, he dies. Dark... I know. Also, the more his quirk activates, the harsher the words get and the more people he must face.
Sure seems easy enough to avoid, but Midoriya is reckless. Despite activating his quirk multiple times, he still is unaware that he has it... How? Well that's something you'll learn soon enough. :>
