Kokichi always had some sort of injury. Whether it be a couple of bruises on his arms, legs, or face, or a few bandages for a couple of cuts, he always had Some injury. Kaito never questioned it, and neither did anyone else. Kokichi was a reckless, and a playful kid who enjoyed being a piece of shit, so obviously he would have a few bruises every now and again.
No one ever questioned it when Kokichi was hurt. But once Kokichi came into class with a broken arm that wasn't there yesterday, his classmates had questions that they wanted an answer to.

My name is Kokichi. Kokichi Ouma. Student from the Mizanaki High School. I'm 16 but I'm by far the shortest boy in my class. And the weakest too.
