After an accident that almost had raph dead and traumatized, he had been hospitalized for 9 years. At some point he gained  the ability to see multiple different colored icons above peoples heads, each color determines how they die. 
His younger brothers Donnie, Leo, Mikey, and Casey have the icons above their heads change color suddenly and randomly raph has to find a way to prevent their fates.
