They got Cas back. And everything should be perfect. Except it isn't. Dean is still a complete mess, and he was so drunk when Cas came back that he thought he made it up. But Cas is still there the next day. And since Dean's a mature adult with numerous apocalypses under his belt, he handles the situation by entirely avoiding Cas.

Cas comes back from the Empty, and Dean can't be normal about it.
