They got Cas back. And everything should be perfect. Except it isn't. Dean is still a complete mess, and he was so drunk when Cas came back that he thought he made it up. But Cas is still there the next day. And since Dean's a mature adult with numerous apocalypses under his belt, he handles the situation by entirely avoiding Cas. Or: Cas comes back from the Empty, and Dean can't be normal about it.

Ever since Dean Winchester got the Mark of Cain on his arm, his world has been turned upside down.
