"Deku.." The child started as she looked up at him. "Why are you in a swimsuit like mine?" She asked simply. Izuku hadn't even considered her not understanding what this was, she was so sheltered from the poor childhood she had. He looked over at her and gave a gentle smile as she continued. "Cause Otosan and Nīsan don't have them on.." He crouched down in front of her and simply smiled.
The fact Eri didn't grow up normally tends to not pass Izuku's mind until she asks him a question after they visit the beach with Aizawa and Mirio and she notices the different attire between the three males.

He brushes it off up until the Hassaikai boss, Eri's grandfather, forces him to make incisions in her arms so that he can take her quirk cells to make quirk destroying bullets that he realises the need to protect her.. was a warning from his own mind that his boss doesn't care she is part of his blood.
