Metatron closed off Heaven and the angels fell to Earth. Dean cannot let go of Sam after he faces death as the tasks to close the gates of Hell would kill him, and he prays to the angels, offering to help the first who complies with his demand. Gadreel, who gains access to Sam's body under the guise of Dean's appearance, now heals him from the inside.

But Metatron had overlooked something, as his actions tore a hole in the fabric of space and time, presenting new challenges for Dean, Sam, and Castiel.
