As in you might be genetically far from each other but still if you share the surname to be specific gotra and your great great grandparents were related then you people are considered cousins. They share a surname, they share the gotra. So it is expected that they consider each other as brother and sister. But when you meet that brother/sister of yours once a year you don't actually believe you are a family.
