Ezra has been going through an extremely rough and traumatizing period throughout his whole life. Everything is hard while to seem like a "normal" person who didn't have to go through the things he had to in his life. His best friend knows a little, but is slowly noticing something is wrong with Ezra when it becomes harder to mask his "abnormalities".
