Philophobia: The fear of love. Love is not something that many fear, however, they should. Love is a powerful, terrifying force that can drive people to do terrible things.
Alexandria Crane loves her husband, even if he is currently rotting in Arkham. But Jonathan Crane aka Scarecrow shows no sign of improving, so Batman does everything in his power to keep Alex and John apart. This only makes John grow desperate until he escapes Arkham to be with his wife. John's escape triggers a landslide of other Arkham breakouts until Gotham is overrun by inmates. This starts a gang war that Alex focuses on surviving while Scarecrow tries to climb his way to the top of the ladder to rule Gotham with the only person in the world he loves... his wife.

Being a psychiatrist at Arkham means dealing with some of the most dangerous criminals in Gotham. Doctor Lilith Matthews was very aware of this, but took the job anyway. She was supposed to be his substitute doctor, until his recovered. Instead she was changed to his full on doctor.
