When you turn 16 you automatically get your soulmate's name tattooed onto your wrist, with no way of removing it or even changing whom your soulmate is; you just deal with it. For Dan, he happens to get his best mates name imprinted on his wrist. His best mate -Phil - being (supposedly) straight, has yet to receive a soul mate. When he does, it's not Dan's name that he gets - it's someone else's.
