In February of 2014 the Sons of Jacob orchestrated an attack on the United States Capitol, the White House, and the Supreme Court that devastated the country, implementing marshal law and racking up a death toll to shame 9/11. By October of 2015, the country founded by the Sons of Jacob, Gilead, has taken over the majority of the Eastern Seaboard as well as parts of the West Coast, instituting twisted and perverse biblical laws that all are required to live by, or be executed.

Sadie was a psychology major at the University of Indianapolis when she was ripped from her dorm room by black-clad soldiers and forced into Gilead's service. For the charges of unwomanly crimes, her only option aside from death is to fulfill her biological and god-demanded purpose of producing children for the powerful and most holy of Gilead's leadership as a handmaid.
