I'd only make one very small adjustment for flow: "Getting out of the streets is hard. But once you do, the world can show you a life you never imagined - a life filled with peace and happiness. But what do you do when the streets come knocking at your door again?" The "again" gives the final line a little more sting because it implies the streets are never truly gone.
