Palpatine overestimates the hold of darkness over Anakin (which might have or not faltered during the battle on Mustafar) and thus makes the mistake of sentencing him on overfeeling the (intentionally) augmented and excruciating pain during the inhuman long surgery. Sidious too blind and power hungered, insists it continues, as more pain will make his apprentice more force powerful and the perfect slave of rage and darkness. Only, the pain was far greater than anything that could matter and Anakin, for yes, he was still Anakin Skywalker at that point, was seeking to end it. End it all. He calls his death with Obi-Wan's name and he finds a way to reach for it (for him). Across the stars and galaxy, across the time and their dead bond, Anakin meets Obi-Wan and his end.
