Kokichi was the antagonist, that was the role he played. That was the role he chose. He never expected to lose the people he cared for. He never wanted to hurt anyone. Why does nobody understand how hard it was for him? To lie and lie, and hurt, to keep all of these feelings to himself.

So, when he wakes up from a four year coma, and it was all a dream, what happens? All of these people he hurt.. were they still there? Did he really hurt them? It's hard to tell reality and dream apart.
