It is 1946 just after WWII. Being prosecuted as a homosexual at this time in history was treacherous, from medication' that is supposed to 'cure' them to execution.
England has been prosecuted and now has to take this medication to be 'cured' he knows it won't work but it is that or the consequences and threats.

Is the clown the fool of the story, or are you? After all, the clown knows it's an act, but the audience is so lost in it that they tend to forget. Alfred had lived a long life.
