Task force 141, with captain John Price as their frontman, has been sent on a mission to investigate illegal activities within an old, to the public empty asylum.
Just as all sergeants clear out every floor, Lieutenant Simon "Ghost" Riley decides to trust his gut feeling and search one of the hallways on the top floor once again, even though his mate has already called it clear.
As Ghost enters one of the last cells in the hallway, he wishes he wouldn't have.
