Basically Izuku has been fused with Kim Dokja and has experienced everything in ORV only for both to finally reach the end of the scenarios and they are swapped with KDJ being fused into Izuku's original body in the MHA world and they both have to live with the loss of everyone in the ORV world....I know bad description but had to write it down before I forgot.
