A guy gets run over by a truck-kun after reflexively saving two children who are about to be run over by a truck, and finds himself in the presence of supreme beings called the constellations. 

These supreme beings tell Eren that by saving the children, he caused the timeline to be broken. Since what Eren did was a good thing, the constellations decide to give Eren the option to be reborn in a different universe. 

As a result, Eren is reincarnated in the world of Naruto with the characters' templates from the world of jujutsu kaisen and is reborn as a baby with the hope of a more comfortable life in his new life.

Opening his eyes to the world of Naruto for the first time, eren learns that he is the son of shimura danzo, but it is not enough to dissuade eren from his dream. 

Eren has already got his first template before he realizes what's happening.
