Andrea Fanti, ex primario di medicina interna, viene catapultato in una realtà difficile da accettare, il destino gli farà cambiare rotta, cambiando drasticamente il suo presente; Agnese Tiberi, la sua ex moglie, diventata ad oggi la sua compagna, viene colpita per la seconda volta dalla Mesotelioma, ed Andrea è disposto a tutto pur di restarle accanto, dando così le dimissioni in ospedale.
Intanto Giulia Giordano, un medico strutturato dell'ospedale in cui lavora Andrea, le viene offerto di prendere il posto di Andrea come primario. Dovrà decidere se diventare il nuovo primario di medicina interna o se trasferirsi a Roma.
