Surprise, surprise, one of Leo's portals didn't work, who would have guessed. But that's not the turtles' main priority right now, because during a fight with Big Mama, Leo DID manage to open a portal, although when Leo and Donnie go through it, they black out. When they wake up however, they wind up staring at themselves through eachothers eyes. Literally.
