What if after Derek's dad was killed his mother moved on. And what if she moved on to his best friend's dad. What if she had more children after that?

What if the Sloan-Shepherd family actually had way more than 5 children, what if it had 10. Starting with Nancy and working all its way down to Jessica.
Nancy, Lizzie, Mark, Derek, Kathleen, Amelia, Lucas, Noah, Elisa and Jessica. But what if Nancy and Kathleen actually weren't Shepherd's biologically, what if they were Sloan's instead. But they always called Carolyn mom, even before their biological mother died.

Lizzie, Derek, and Amelia are the children of Carolyn and her husband, Nancy, Mark and Kathleen were adopted by Carolyn at the age of 12 when she married their father Andrew Sloan and then together Carolyn and Andrew had Lucas, Noah, Elisa and Jessica, ending off the family with the last four children.

Now 4 of them still work where they grew up, but the other 6 have moved to Seattle and somehow ended up working at the exact same hospital.

But not right away. It was Carolyn's first time visiting her children in Seattle since her second husband died, but the visit didn't go as amazing as they had hoped, as Carolyn ended up getting admitted to the hospital.
