Y/n was dealing with a sickness, they'd been coughing up blood and didn't tell anybody as not to worry them.
One dah while out with the neighbors on a picnic, y/n climbed a tree with sally and Julie. They started to cough blood and pass out, hitting their head on the ground from the tree.
